About Our Services
As a Psychiatric Mental Health Nurse Practitioner (PMHNP), I specialize in evaluating, diagnosing, and treating a wide range of mental health conditions in individuals aged 13 to 64.
​​If you are transferring care from another provider or seeking a second opinion, you will need to complete a Release of Information (ROI) form. Please note that transferring care does not guarantee the continuation of your current medication(s); this will be discussed and determined during your initial appointment. Please give us a call if you have any questions about our prescribing practices before making an appointment.

We do not provide short-term disability, long-term disability, or court-related documentation. Medical forms such as FMLA requests, ESA letters, school letters, and other miscellaneous forms will only be considered for established patients who have been with us for at least 3 months, with rare exceptions. There is a fee for these services, and insurance typically does not cover these charges.

​As a solo practice, we do not have on-call providers and therefore, we do not offer emergency services. If you are experiencing an urgent mental health concern that cannot wait until the next business day, please contact emergency or crisis services.

For existing patients, we do offer the option to request an "urgent" appointment during business hours. Requests will be evaluated based on the urgency of the situation and provider availability.

​Cancellations-No Shows-Rescheduling
For your convenience, we offer online scheduling. Once your appointment is confirmed, you will receive a link to our Online Intake Form. It is essential that the form be completed within 24 hours of receipt; otherwise, your appointment will be canceled to make the time available for others. If you don’t see the email, please check your Spam or Promotions folder, or feel free to contact us.

We ask that you provide us with at least 24 hours notice if you need to cancel or reschedule your appointment, so we can offer that time to another patient. You will be financially responsible for appointments not cancelled or rescheduled with appropriate notice.
We generally do not offer to reschedule no-shows for the initial evaluation. Patients who cancel less than 24 hours before an appointment, or fail to attend will be charged a $100 fee ($200 fee for new patient appointments). A "no-show" is defined as missing an appointment without providing 24 hours notice or arriving more than 10 minutes late.
